Note: It is to be noted that in the Eastern Region (in Kolkata VFS): From March 01, 2012 onwards the visa fees will be accepted only in the form of CASH.

• Application form completely filled and signed by applicant. The application form must be printed on A4 size paper and the print should be taken in back to back page.
• One latest passport size colored photograph (35 x 40 mm wide) with white background, 70-80% face coverage. The photograph should be taken with a digital camera with high quality colour and printed on photo-quality paper. The photograph shouldn’t be older than 6 months. Applicants must note that the photograph should be pasted, not stapled.

Applicant’s Passport / Travel Document must:

• Have been issued within the last 10 years, and its validity must not exceed 10 yrs.
• Should have a validity of 3 months beyond the intended stay.
• Have at least 2 blank pages.

Applicants should have an overseas medical insurance with minimum coverage of Euro 30,000/- for emergency hospital and repatriation expenses. (Photocopy should be submitted). The list of some of approved Indian Travel Insurance Companies offering Travel Medical Insurance for Schengen Visa is as follows:

• APOLLO MUNICH (for the age group of 6 months to 70 years).

In case of Applicants applying at Eastern region (in Kolkata VFS):- Travel Health Insurance submitted for all applications must be valid for 15 days beyond the intended duration of stay in the Schengen Area.

Other Requirements:

• The travel itinerary with travel dates, including a return flight reservation or bookings. Applicants must mention if any other country or countries will be covered during the whole trip.).
• A set of photocopy of Applicant’s Passport.
• Documentary proof of the economic status of the Company, which is sending the individual abroad.
• Registration Certificates of Company or Proof of Board Membership for liberal professionals or Letter from Medical Institutions (in case of doctors).
• Bank statement of the past six months.
• IEC number (if applicable).

Application form should be enclosed with a letter from Indian Company or Institution, mentioning the purpose of the visit, the professional status of applicant, and details related to activities and business relation with Italian company. The letter should be signed by the authorized person, whose name and charge must be clearly identified.

Documentary proof of availability of sufficient financial means of the applicant to survive in the country:

• Salary slips / certificates of the past three months and Form F 16.
• Status of personal bank account for the past six months, in original letter from the bank.
• Income tax Challans (of last three years) and Form 16
• Foreign exchange endorsement on the passport and receipt.
• Credit card copy with statement of credit limit.

Original letter of invitation from Italian company in Italian (should be attached with the form), duly signed by the authorized person, along with photocopy of the pages of his / her picture ID (passaporto o carta d’identita) giving relevant data, signature and validity of the document.

The processing time is up to 10 to 15 working days. The Embassy reserves the right to request to inquire about other documents.

Supporting documents: verification of original documents; Import - export license, import-export invoices to Italy, PAN card etc.

Please note: Photocopies are compulsory for all of the original documents to be returned. According to law in force, the Embassy / Consulate reserve the right to ask for further documentation. Applicants can always be asked to appear for a personal interview.For frequent travelers, photocopies of previous visa issued in the last two years by Schengen Countries, UK, USA, Ireland, Australia, New Zealand or Switzerland are required.

In case of minor children (application to be signed by both parents), an approved Leave Application from school needs to be submitted along with the application form. If the minor is traveling unaccompanied or with any one of the parents an "Affidavit" attested by the relevant Home Department (as per their place of residence) should be enclosed with the application form.Supporting Documents:Verification of original documents; i.e. Ration card (original and English translation), duly attested, in case the Applicant is the parent of the inviting person living in Italy. If any change has occurred in this regard then an adequate evidence of residence should be submitted (telephone / electricity bill etc.). In addition, it should be noted that additional documents may be required in certain cases. Moreover, applicant could be asked to appear for personal interview.

Charges: INR 510/- (Inclusive of government service tax 12.36%) only for Cochin Applicants.

Courier Charges: INR 300/- (Inclusive of government service tax 12.36%).

Form filling Charges: INR 102/- (Inclusive of government service tax 12.36%).

The Banking Instrument must be payable favoring 'VFS GLOBAL SERVICES PRIVATE LIMITED'. It should be noted that one Banking Instrument will be accepted per applicant. Alternatively applicants applying at VFS Delhi & Jalandhar have the option to pay visa fees in cash also for which service charges of INR 57/- (inclusive of service tax 12.36%) per application will be applicable.

There is a service charge of INR 674 /- levied per application over and above the visa fees. The courier facility is available at INR 300 /- per application & SMS facility is available at INR 81/- per applicant (optional) & Form filling Charges: INR 102/- (Inclusive of government service tax 12.36%).

• Transit Visa is required. Visa is issued for a maximum stay of five days and is subject to possession of travel ticket and Visa for the country of final destination.
• Application form completely filled and signed by applicant. The application form must be printed on A4 size paper and the print should be taken in back to back page.
• One latest passport size colored photograph (35 x 40 mm wide) with white background, 70-80% face coverage. The photograph should be taken with a digital camera with high quality colour and printed on photo-quality paper. The photograph shouldn’t be older than 6 months. Applicants must note that the photograph should be pasted, not stapled.
• Confirmed Visa for the next destination after Italy.
• Confirmed travel ticket of the final destination.
• Documentary proof of confirmed and prepaid hotel reservation.

In case of minor children (application to be signed by both parents), an approved Leave Application from school needs to be submitted along with the application form. If the minor is traveling unaccompanied or with any one of the parents an "Affidavit" attested by the relevant Home Department (as per their place of residence) should be enclosed with the application form. Supporting Documents: Verification of original documents; i.e. Ration card (original and English translation), duly attested, in case the Applicant is the parent of the inviting person living in Italy. If any change has occurred in this regard then an adequate evidence of residence should be submitted (telephone / electricity bill etc.). In addition, it should be noted that additional documents may be required in certain cases. Moreover, applicant could be asked to appear for personal interview.

The photograph should be taken with a digital camera with high quality colour and printed on photo-quality paper. The photograph shouldn’t be older than 6 months. • Confirmed onward/ return air ticket.
• Onward visa (if any).
• Documentary proof of hotel confirmation from Italy.
• Application form should be enclosed with a covering letter from Applicant explaining the purpose of trip, the duration of stay and outlining the travel itinerary, photocopy of the income tax clearance for the previous year.

Evidence of funds in form of foreign exchange endorsement on the passport and receipt.
Copy of Credit card with statement of credit limit or bank guarantee ("fidejussione bancaria") from the sponsor in Italy (original and photocopy),
Proof of sufficient financial guarantees (original and photocopy to be presented):

Confirmed leave letter from the employer.
Indian seamen travelling on duty must carry a Letter of Guarantee from the shipping company with seamen book and passport.
A fax message from the shipping company in Italy (duly attested by the competent Port Captain) to the Embassy mentioning the particulars of the seamen, date and port of joining the ship is needed.
In case of a family visit, sponsorship letter attested by the Foreign Registration Office of the Italian Police Station (Questura) of the locality of residence of sponsor must be attached with visa documentation. The Visa is valid for 90 days and issued for a maximum stay of 30 days in the country.

• “Dichiarazione di Valore” (Declaration of Value) of the applicant’s educational certificates, duly legalized by the Consular Office of this Embassy. The local Institution or School should advise this Embassy directly (in case) it is not required.
• All education certificates duly Apostilled by Ministry of External Affairs.
• Documentary proof of acceptance of enrollment by the Italian School or Institution, mentioning the duration of course.
• Receipt of full payment of Tuition fees (if needed).

Documentation related to the applicant’s economic status:
University Courses or Post graduated courses: Evidence of means of support in Italy of not less than 350, 57 Euro for each month of the academic year. This availability can be proven in the form of: Bank Statement of applicant or family OR Bank loan.
Scholarship from Italian Government: Proof of scholarship (in original). Where the amount of the scholarship doesn’t satisfy the minimum established by law (see annexed table) the availability can be proven through: Bank Statement of applicant or family OR Bank loan OR Bank guarantees.
Non-University courses: Applicant’s adequate financial guarantees OR Bank guarantees (fidejussione bancaria”) covering the expenses for the duration of stay according to the foreign exchange value. Also, adequate means for repatriation OR return flight ticket or booking (for courses for period up to 90 days).

• Applicant’s adequate evidence of Boarding and Lodging (if required).
• Confirmed return flight ticket or booking (for courses up to 90 days).
• Insurance coverage for medical treatment and / or hospitalization (not required for admission to Italian Universities).
• (For visa requested over 90 days) the applicant need to submit his / her original (not scanned) admission letter stapled with the passport. At the time of collection of the passport, applicant must make sure that the admission letter or the declaration letter by the university is duly stamped by the Embassy.
